Q: How is this different than what I can find on PeeringDB?

A: PeeringDB is a fantastic source for the raw data. Q: Does the data differentiate between extension nodes and IX's based in a state?

A: No. Currently PeeringDB's data makes no differentiation between the two. Read the discussion on LinkedIn

Q: Is it possible for an Internet Exchange in one state to serve a city or area in another state?

A: Yes. This can be done either through distributed core nodes or extension nodes. A good example of that can be found here (LinkedIn conversation about New Jersey and New York City) and the difference between core and extention here (LinkedIn conversation)
